Direct Support Professional 2nd, 3rd shift -Qualifications – High school diploma or equivalencySome experience in human service fieldSkillsEffective written and observational skillsGood interpersonal skillsAbility to work with team approachAbility to work with people at different functioning levelsGood problem solving skillsAbility to accommodate flexible work scheduleFamiliar with computer programs (e.g. Word, Excel)The ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighing up to 40 lbs. Ability to legally operate a vehicle in NY State
MAJOR FUNCTIONS
Writing or assisting in writing reports and documenting individual progressParticipate in team planning meetings and case reviewsTeach/assist in developing/maintaining ADL skillsAct as a role modelPerform routine site household maintenance and upkeepAssist in teaching self-medication skills, utilization of healthservices and proper nutritionAdminister/supervise medications and/or treatments as per doctor’s Provide lift, transfer, and assistance with range of motion exercises as needed Plan, coordinate and assist individuals in identifying personal recreational interestsProvide appropriate support to meet individual needsPlan/prepare meals as neededMaintain case records and provide documentation according to program standardsIntervene appropriately to assist in crisis managementUnderstand and follow all program policies and proceduresReport all household or resident sensitive situations to supervisorParticipate in and successfully complete all required staff training and development activitiesMaintain personal property listsParticipate in intake and discharge planningAssist individuals in the development of decision-making skillsCommunicate with other service providersProvide transportation as neededAssist individuals in accessing community servicesRepresent the Agency and program in the community and with other service providersEstablish relationship with all residents and families as appropriateRegular and substantial contact with individuals receiving supportPerform other related tasks as directed by supervisorIn compliance with all applicable government laws, rules, regulations and standards and all Catholic Charities Policies and Procedures the duties and responsibilities of this position include but are not limited to the above.
