JOB DESCRIPTION
As a Community Habilitation Professional you will be working as a partner with individuals with developmental disabilities to support community integration and independent life style. The position is primarily one of being a catalyst to become a bridge builder between the community and the people we support.
RESPONSIBILITIES
Provide in home & community Habilitation Services to individuals who have I/DD Provide oversite and delivery of community services
REQUIREMENTS/SKILLS
Valid, clean driver’s license with one year driving experience required Ability to work as part of a team, problem solve, and demonstrate a working knowledge of person centered approaches Excellent verbal and non-verbal communication skills, interpersonal skills and organization skills We generally work Monday thru Friday with flexible schedules between 8a – 6p . Based on the needs of the participants No Mandations
