Individual Support Coordinator Nurture Home LLC Halifax, PA 17032 Overview We are seeking a compassionate and dedicated Direct Support Professional to join our team. In this role, you will provide essential support to individuals with disabilities or special needs, helping them achieve their personal goals and improve their quality of life. You will work closely with clients in various settings, including long-term care facilities and home care environments, ensuring they receive the assistance they need in a safe and respectful manner. Responsibilities Provide direct support to individuals in daily living activities, including personal hygiene, meal preparation, and medication management. Assist clients with mobility and physical tasks, including lifting when necessary. Monitor and document vital signs as required, ensuring the health and safety of clients. Maintain a clean and organized living environment for clients, adhering to hygiene standards. Implement individualized care plans developed in collaboration with healthcare professionals and family members. Uphold confidentiality and adhere to HIPAA regulations while providing care. Foster positive relationships with clients through effective communication and social interaction. Participate in training sessions to enhance skills in CPR and other relevant areas of direct support. Skills Experience in direct support or long-term care preferred; home care experience is a plus. Knowledge of CPR techniques and ability to perform basic first aid. Strong interpersonal skills with the ability to assist clients compassionately. Familiarity with monitoring vital signs and understanding basic health needs. Attention to detail in maintaining cleanliness and organization within the client’s environment. Understanding of HIPAA regulations to ensure client confidentiality is maintained. Background in social work or related fields is advantageous but not required.
