Job Description
We are currently seeking Supports Service Professionals (SSP) to work with an individual with special needs in the Hershey area.
Days:
Monday
- Friday (5 pm
- 9 pm)Saturday
- Sunday (7 am
- 9 pm) Qualifications
- Experience in the Home Health Field-Background Checks Clearances Ideal Candidates will be expected to
- Accurately follow an individualized plan of care-Monitor all aspects of a patient’s care-Consult and coordinate with family and healthcare team members to plan, implement, and evaluate patient care plans-Maintain accurate, detailed reports via an electronic medical record
- Provide hands-on care to the individual and assistance with personal hygiene, activities/ outings, social engagement, shopping, planning, cooking, and cleaning.
- -Help with getting to community activities and developing relationships
- Complete Orientation and Training-Complete Annual Training-Be patient, respectful, trustworthy, and able to encourage
