Responsibilities:

We are looking for a qualified individual who enjoys making a positive difference in the lives of others. As a direct care worker, you will assist in daily living activities, including health and hygiene management, cooking, cleaning, recreation, and documentation which include shift, incident (if applicable), and individual progress reports. On a daily basis, maintain a safe and therapeutic environment that meets the emotional, physical, and personal needs of each individual we serve. Provide services/support to individuals Assist individuals with developing socialization, meal preparation, and personal hygiene skills. Transport individuals to medical appointments, personal shopping, and recreation activities. Cultivate safe and supportive relationships via natural supports. Coach individuals with identifying and facilitating volunteer opportunities or paid employment. Complete necessary training based on the Individual’s needs. Accurately completes all in-house documentation and reports of individuals’ progress Conduct intake assessments and coordinate referrals. Ensure implementation of individualized support plans (ISP). Manage ongoing case management and client support. Enter and finalize incidents (does not have to be a CI). Ensure compliance with IDD regulations and report client needs to the Director. A direct service worker shall be responsible for the daily care, training and supervision of individuals.
