Are you a warm, caring, and dedicated individual who is passionate about helping others? Concordia Hospice Of Washington, is seeking a Hospice Home Health Aide with a big heart and a desire to make a difference in the lives of patients and their families during their end-of-life journey.
Position Summary for the Home Health Aide
The Hospice CNA provides personal care and assists in activities of daily living and related services to hospice patients in the inpatient hospice facility. This employee functions under the direction, instruction and supervision of the Registered Nurse (RN) Charge Nurse on duty.
Qualifications for the Home Health Aide
Ability to understand and carry out directions and instructions and maintain appropriate records
Current Heart Saver First Aid certification
Excellent organizational skills with attention to detail
Outstanding communication skills (both written and verbal)
Reliable independent means of transportation with a valid Pennsylvania driver’s license and at least the minimum state required vehicle insurance
Responsibilities for the Home Health Aide
Provides personal care according to the established individualized plan of care as developed by the RN member of the hospice team such as (but not limited to): bathing, grooming, dressing, shaving, feeding; checking vital signs; assisting with ambulation; changing position in bed; assisting with transfers, changing bed linens; assisting with elimination; providing oral hygiene, skin care, foot care, ear care, light house cleaning and meal preparation
Completes inpatient facility housekeeping duties including (but not limited to): laundry, cleaning, minor meal preparation
Completes all computer and/or paper documentation requirements within the timeframes specified
Participates in patient and/or family teaching activities as directed by the RN hospice team member
Together with the hospice team, provides care as established by the IDG to meet the physical, emotional and spiritual needs of the patient as necessary, and appropriately supports the family at the time of death
Maintains regular communication and interaction with patients, families, physicians and hospice team members
Participates in required on-call responsibilities as assigned
Attends and participates in staff meetings and other related activities when required
Completes required in-services; ensures all required continuing education credits are obtained for certification renewal if applicable
Demonstrates an understanding of the Quality Assessment Performance Improvement (QAPI) process, hospice policies and procedures, and participates in projects when requested
Participates in the orientation of new staff members when requested
Commits to high ethical standards and complies with the Code of Conduct, state and federal regulations, accreditation standards, and Concordia Hospice of Washington policies and procedures; maintains confidentiality of patient and employee records and information; has a duty to report compliance concerns, significant legal risk questions, fraud, abuse, suspected or actual misconduct to the Executive Director and/or Compliance Manager
