Homecare Hospice Aide (Horry County)Job Summary Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with our Service Excellence Standards and Core Values. Presents with a positive attitude to patients, families and Hospice Team Members in the home and in the Hospice Office 100 percent of the time. No more than two complaints in a year made by patients/families/team members that are evaluated by the Clinical Manger and considered justified. Is always clean and neatly dressed as per department dress code. During home visits, personal care is given in a calm, comforting and professional manner. Provides safe, effective, personal care and light housekeeping services to Hospice patients, in their place of residence, in accordance with the plan of care, as assigned by the primary RN or Designee. Notifies the patient’s primary nurse and/or manager of patient’s change in condition, family concerns or problems as instructed. Recognizes signs and symptoms of the actively dying patient and communicates this information to team members. Performs pulse and/or respirations check, when requested per RN and reports findings to RN. Performs routine skin inspection and reports any abnormalities to RN. Performs non-sterile dressing changes, routine decubitus care, Foley catheter care, removes and replaces colostomy/ureterostomy bags per RN instruction and after having demonstrated competency to RN. Uses medical equipment as directed by RN or physical therapist and assists patients with ROM exercises as prescribed and according to care plan. Assist patient and family to meet social and emotional needs in accordance with goals and objectives of hospice care. Encourages independence, whenever possible. Makes assigned bereavement visits as indicated by Clinical Manager. Actively participates as a Hospice Team Member, attends staff meetings, in-services and other meetings as assigned. Contact information and services provided are documented accurately and in a timely manner as per department policy. Laptop is synchronized every morning and every evening to assure all information is up to date. Demonstrates continual job growth by obtaining 12 hours of continuing education annually, including all mandatory training. Keeps informed of all new or updated policies and procedures. Completes age specific CBTs annually; recognizes the normal signs of growth and development in each age group served (indicated above) and understands the requirements of assessment, treatment and care of the specific age group. Qualifications /

Training:

Current South Carolina Driver’s License and Automobile Insurance as required by law. Must be empathetic, responsible, able to use time effectively, willing to accept supervision and relate well to patients and families. Must pass a competency skills checklist during orientation

Licenses/Certifications/Registrations/Education:

Minimum of a High School Diploma/Equivalent from an accredited school required. Completion of a 75-hour Nursing Assistant Course with a minimum of 16 hours of supervised practical training, and present a certificate or diploma or State Aide License.,
