The DirectSupport Professional is responsible to provide direct supportwithin a single program or unit within a program, typicallysupporting 4 – 6 individuals in residential settings, or up to 10individuals in periodic programs. The Direct Support Professional is responsible to provide directsupport or assistance in accordance with individual service orprogram plans and may include socialization, health maintenance,medication administration and skill or behavioral development. TheDirect Support Professional assists with day-to-day activitiesincluding personal care, housekeeping, recreational activities,transportation, community integration, shopping, citizenship, andother activities of daily living. The Direct Support Professional generally works 16 to 40 hours perweek as scheduled. The Direct Support Professional works at theprogram location and may accompany individuals into thecommunity. Attends orientation and on-going training as directed; participatesin monthly staff meetings. # stays on shift until replacement arrives; may fill in for othershifts where required without causing unapproved overtime, or inthe event of emergency. # Complies with all established safety policies, procedures andpractices and reports all work-related injuries to his/hersupervisor. – Services and Supports # Provides direct support or assistance in accordance withindividual service or program plans; assists with socialization andbehavioral development, personal care, housekeeping, recreationalactivities, transportation, community orientation, shopping,financial management, citizenship, and other activities of dailyliving. # Accurately documents progress and activity; accurately and timelycompletes billing documentation as applicable. #
Management of Individuals’ Assets:
Assists with money management,as assigned, and assures safety of individuals’ funds andproperty. # implements Network Customer Service Standards. # reports medical, behavioral and other incidents in accordancewith Network policy and external requirements. # Health Care # May accompany individuals to medical appointments; relays ordersand information to and from medical providers as required. # If assigned, accurately administers and documents delivery ofmedications and treatments; documents health concerns; followsindividual health care directives. # Correctly follows procedures in utilizing medical equipment;Prepares or supports individuals to prepare food in accordance withplanned menus and individual choice; May transport individuals intothe community; drives safely and according to local laws; assuresproper use of safety equipment including seat belts, lifts andwheelchair ties; reports accidents to appropriate authoritiesimmediately; reports accidents and safety concerns immediately tosupervisor or maintenance personnel. # Performs daily and seasonal housekeeping as directed; may performyard maintenance including keeping walkways safe and snow removal;reports equipment or facilities requiring repairs to supervisor ormaintenance personnel as required. # Checks water temperature as required when assisting with bathing;Employee may from time to time need to use a personal cell phonefor business purposes and will be provided a stipend to cover thebusiness costs of doing so. Six months of experience in human services preferred.
Certificates,Licenses, and Registrations Required:
Car registration and vehicle insurance if providing transportationfor individuals receiving services. All state-required training and certification completed in mandatedtimeframes. Job requires performance of duties that involve potential forexposure to blood, body fluids, or tissues. Travel required asneeded If an employee is determined to have lost their driving privilegesby failing the company MVR driving record requirements per companypolicy, the employee will be required to sign a Non-Driver form andrestricted from driving on behalf of the company until their MVRreport satisfies the company requirements for a good drivingrecord. – – AMERICANS
