Programming Outcomes:
Support individuals in meeting their personal outcomes
- Monitor and record data on personal outcomes
- Supporting Roles (for the individuals): Provide the appropriate level of support the situation requires according to individual support plan or health status
- Support individuals in choice making Support and assist the individuals in their work, social and leisure activities
- Support and assist community integration
- Support family relationships Remain on shift until the relief person arrives
- Be attentive to and report any health and safety concerns of individuals
- Ensure the safety of all individuals in case of emergency events, such as fire, tornado, and armed intruder
- Follow emergency procedures accurately and promptly, such as CPR, First Aid, Heimlich Maneuver, and Crisis Prevention Intervention (CPI) Observe and record accurate information on seizure reports
- Practice Universal Precautions
- Support individuals at healthcare appointments
- Support positive life style choices Administer medications accurately and in accordance with agency policy and procedures according to individual program plan
- Physical ability to assist in Hydrotherapy Pool (hot and humid conditions) and recreational pool according to individual program plan
- Keep all chemicals and cleaners securely locked up
- Complete all required documentation per shift
Personal Hygiene:
Perform all personal hygiene in a private setting
- Assist in bathing, hair care, dental hygiene, and shaving
- Keep personal care boxes stocked, clean and in order
- Assist in fingernail care
- Assist with dressing and undressing
- Assist with individuals selecting appropriate, clean, seasonal clothes
- Assist with toileting and changing adult briefs
- Utilize mechanical lifts according to individual program plan
- Complete all required documentation per shift
Physical Requirements:
Utilize proper lifting techniques in all areas
- Ability to lift, move, and position individuals according to Physical Therapy guidelines according to individual program plans
- Ability to bend, stoop, and squat
- Ability to push, pull, reach, and stretch
- Ability to run as needed in emergency situations
- Ability to work in noisy environments, indoor and outdoor as needed

- Community, work and classroom areas: Sweep, mop, vacuum, dust daily Empty trash and recycling containers daily
- Clean trash cans as needed Take trash cans to street or dumpster
- Maintain perimeter of worksite (e.g. classroom hallways, patios, driveways, carport)
- Disinfect common and shared surfaces and items daily
- Be alert to and report maintenance needs and safety concerns
Maintain Equipment:
Clean and care for wheelchairs and cushions daily

- Job Description Addendum Community Volunteer Location Supervisor Addendum Effective February 6, 2017 Monitor, manage, and supervise DSPs to ensure expectations are clear and training is current Monitor bus duty system to ensure efficiency and safety Serve as the second bus checker if the site is the vehicle’s last stop.
Plan Person Centered curriculum that focuses on increased independence, employment, and community involvement. Train and monitor DSPs on and assist in implementation of curriculu111 and outcome/action step training and the documentation of each. Provide monitoring and oversight to DSPs and individuals at n1ealtimes Serve as mealtime card and Behavior Support Plan trainer. Monitors check in/out system to ensure individuals are accounted for. Monitor system for supporting individuals with hygiene Monitor system for supporting individuals with behavior management issues. As needed, communicate any issue an individual may have to the SPC/Case Manager. Assure DSPs report and note unusual events and assist with completion of yellow and white incident reports, seizure reports, behavior reports, etc. Fill in roles as requested by Coordinator. Maintain necessary supplies and a cleaning assignment/documentation protocol. Ensure Person Centered language and practice and advocate to maxin1ize individual com1nunity inclusion and satisfaction. Ensure that emergency procedures are known and followed by all staff. Ensure that the emergency contact number list is accurate and available to all staff. Serve as liaison with church personnel. Information must be approved by the supervisor before changes are requested. Any complaint by the church personnel should be reported to Coordinator. Manage volunteer opportunities for individuals within the community site Responsible for maintaining the security of the site (opening and closing each day) and keeping access code confidential. Responsible for ensuring that all items that need to be locked up, such as electronics, documents containing inforn1ation covered by HIPPA, etc. are secured when not in use. Acquire and keep current OGC medication administration status. Monitor the curriculum and other supplies and notify Coordinator when more are needed, ideally in advance of running out. Ensure that personal cell phones are not used by the staff at the site. Manage individuals con1ing and going so that if scheduled for an event, they are available for it. Monitors time off requests by DSPs and calls in substitutes. Ensures that 1:3 staff ratio is maintained. Reports to Coordinator Meets with team regularly to ensure smooth site operations. Work collaboratively with SPC/Case Manager as well as CP, Employment, Residential and Families.
