  • SILP works to support, encourage, andteach independence in all areas of daily life to adults with adevelopmental disability.
MAJOR RESPONSIBILITIES
  • HOME SUPPORT:
  • The Direct Support Professional
  • Supported IndependentLiving ProgramTeaches daily living skills in the consumers homes and inthe community.

Assist consumers with health care, finances, cooking,shopping needs, home care, personal care, and community education. Rolemodel and provide education with healthy relationship skills andboundaries. Serve as a consumer advocate with medical, dental, legalprofessionals, and other service providers, as needed. Monitor withproficiency self

  • medication administration, as needed. Demonstrateability to respond to consumer emergencies and provide emotionalsupport, as needed.
  • COMMUNITY SUPPORT
  • The Direct Support Professional
  • SupportedIndependent Living Program advocates on behalf of consumers with family,peers, and community members, as needed.

Introduce consumers tocommunity resources and facilitate usage. Encourage and supportparticipation in community events. Teach consumers safety skills in allaspects of community and home life. Teach, instruct, and role modelappropriate community and social behavior and expectations, as needed.

Participate as a contributing member of the Supported Independent LivingProgram, Developmental Services, and UCS teams.

KEEPING / ADMINISTRATIVE
  • The Direct Support Professional
  • Supported Independent Living Program submits daily time sheets usingcorrect codes for services provided in our electronical medical recordsystem.

Request and submit all necessary documentation from otherservice providers and professionals. Complete progress notes, monthlysummaries, and all relevant d cumentation in a timely manner. Maintainawareness with all relevant consumer goals and supports as stated in theISA, and document progress. Attend staff meetings, supervision, andscheduled agency/division meetings, as required. Attend all necessaryrequired trainings.

  • REQUIRED QUALIFICATIONS
  • High School Diploma or equivalent. Relevantcollege courses preferred. Computer skills including e-mail usage.Valid Drivers License.
