Job Summary:
Provides nursing care, under thedirect supervision of a Registered Nurse or physician, inaccordance with the legal scope of practice and within establishedstandards of care, policies, and procedures. Demonstratesperformance consistent with the Medical Center Vision, Mission, andStrategic Plan of the organization. Remains flexible to changingsystems; is expected to demonstrate quality and effectiveness inwork habits and clinical practice; and treats co-workers, patients,families, and all members of the health care team with dignity andrespect.
Essential Responsibilities:

- Adheres to hospital standards, which customize care for age-relatedneeds of the population served.
- Keeps health care team members’informed regarding patient’s responses to interventions.
- Collaborates with registered Nurse to develop individualized planof care that reflects clinical findings, planning, implementing,and evaluating the outcomes of that plan.
- Monitors patients’progress based on the plan.
- Assures care meets standards ofpractice.
- Collaborates with Registered Nurse to assure completionof the discharge plan including discharge barriers andpatient/family education.
- Documentation
- Charting is legible,dated, and timed.
- Documentation reflects thorough data collectionwithin the scope of practice, incorporates physical andpsychosocial findings, diagnostic tests, learning needs, dischargebarriers, multi-disciplinary education plans, and age appropriatecare from admission through discharge.
- Documentation reflectsthat the individualized teaching plan is followed; appropriateMultidisciplinary Patient Education Plans are implemented withinthe scope of practice.
- Clinical Outcomes
- Applies standardprecautions; maintains a safe environment for self and others.
- Discusses patient findings and progress toward outcomes withRegistered Nurse, physicians, and other members of the health careteam.
- Demonstrates competencies during the probationary periodand ongoing by completion of departmental competency validation.
- Participates in departmental performance improvement activities,i.e., planning, and measuring/monitoring, assessing and improving.
- Utilizes computer systems effectively and efficiently.

- Practices customer service standards as defined by theService Area, Medical Center, and specified department.
- Anticipates the customer’s desires, takes proactive steps to meetthose needs, by listening and taking responsibility within scope toensure issues are resolved.

- Promptly answers call lights, alarms, and patientrequests.
- Under the direction of the Registered Nurse, givespatients and their significant others the information needed withintheir scope of practice.

Basic Qualifications:
Experience
- Minimum of six (6)months acute care and/or related outpatient experience within pastfour years.
- Per the National Agreement, current KP Coalitionemployees have this experience requirement waived. Education
- HighSchool Diploma/GED License, Certification, Registration
- Vocational Nurse License (California)

Basic Life SupportAdditional Requirements:
- Ability to read and comprehend simpleinstructions, short correspondence, and memos. Ability to writesimple correspondence. Ability to effectively present informationin one-on-one and small group situations to customers, clients, andother incumbents of the organization.
- Must be willing to work ina Labor Management Partnership environment.
- Also refer toPosition Specifications outlined in the appropriate collectivebargaining agreement.
- Experience requirements may be waived ormodified when an appropriate program or course is available andsuccessfully completed for New Graduate LVNs or for area ofspecialization
- Additional certification maybe required based upondepartment needs within six (6) months of hire (PALS, ACLS, NALS).
