- Direct the day-to-day functions ofnursing assistants and room attendants.
- Complete and maintainmedical forms, reports, evaluations, studies, charts, etc.
- Transfer and discharge Residents.
- Ensure nursing personnelprovides proper Resident care and performs their duties inaccordance with written policies and procedures.
- Participate inperiodical reviews of Residents charts
- Participate in surveys andinspections by Government agencies.
- Interpret policies andprocedures to personnel, Residents, families and visitors.
- Reportpromptly to Social Services and the all complaints or grievancesstated by Residents or Resident family members.
- Supervisenon-licensed nursing staff’s charting activities to ensure therecords are complete and accurate.
- Participate in reviewing andevaluating staffing requirements.
- Record nursing personnel’sabsenteeism/failure to report.
- Make daily changes in staffschedule as indicated by absence or illness and obtain replacementswhere needed
- Make compliance rounds every shift and place them inthe book(s) kept by each unit.
- Ensure that personnel, Residents,and visitors follow established policies and procedures.
- Assistin orientation of new nursing personnel. Nursing Functions
- Administer professional services and provide direct nursing care asnecessary.
- Report to nursing staff new/re-admissions, residentstatus change, resident transfers.
- Ensure that rooms are readyfor new/readmits and staff greets the resident upon arrival.
- Report all behavior changes/ problems to the DON and SocialServices.
- Make rounds on all Residents to observe and evaluatefor changes in Resident physical and emotional status.
- Consultwith Resident’s physicians regarding care, treatment, diagnosticand therapeutic services, and notify physicians of anaccident/incident involving a resident, changes in condition orwhen a Resident expires.
- Make rounds with physicians ifrequested.
- Monitor and check food brought into the facility byfamilies and visitors to ensure that it is within the resident’sdietary allowances.
- Notify sponsors of changes in Resident’scondition, accident/incidents, diagnostic and therapeutic servicesand Resident’s death.
- Complete forms and chart allaccidents/incidents involving residents or employees ands submit asrequired by policies and procedures.
- Assist with arrangingtransportation for Residents that are discharged, residents thatrequires emergency evaluation by a physician, or Residents thatrequire a physician visit, test, or other services that cannot beperformed in the facility.
- Ensure post mortem polices andprocedures are followed and arrange for transportation to thefuneral home.
- Review medication administration records, physicianorders, treatment orders, diabetic flow sheets, and Coumadin flowsheets for completeness, accuracy, and adherence.
- Perform woundcare as necessary.
- Perform tube feedings/and assist residentsduring meal times as necessary.
- Perform medication pass asnecessary. Verify resident’s identities to ensure that prescribedmedications are administered to the proper residents
- Countnarcotics at the end/ beginning of shift to ensure narcotic recordsare accurate. Notify the DON of all drug and narcoticdiscrepancies.
- Ensure proper disposal of drugs and narcotics perfacility protocol.
- Order prescribed medications, supplies, andequipment as necessary per physician’s orders.
- Sign, date, timeand perform all routine charting and record keeping in accordancewith established policies and procedures.
- Complete all requireddocumentation following accidents/incidents involvingresidents/employees and submit according to policy and procedure.
- Properly record and process all verbal, telephone, faxed andwritten physician’s orders to resident’s charts, MAR’, and TAR’s.
- Communicate diet changes with dietary (using dietary communicationforms), giving dietary the white copy and placing yellow copy inthe physician’s orders.
- Communicate all discrepancies notedconcerning physician’s orders, diet orders, omitted medications,charting errors, etc.
- Assist the care plan team in identifyingall nursing/ medical goals and needs of each Resident to provideand implement a comprehensive care plan.
- Ensure all nursingpersonnel are aware of the care plan for each Resident and it iscommunicated via the cue card in each Resident’s closet.
- Participate in the weekend rotations on the medication nurseschedule assuming those responsibilities of a charge nurse in theabsence of a nurse supervisor.
- Assist with assessing, planning,and implementing plans of care for wound prevention, ADL functions,plan of care and Resident compliance.
- Assist with the bodyassessments for new admissions, re-admissions, and when significantchanges are documented Quality Assurance Functions
- Attendappropriate nursing staff meetings.
- Participate in the QualityAssurance Committee and in other sub committees as deemed necessaryby the Administrative Staff.
- Follow hand sanitizing, PPE andUniversal Precautions guidelines,
- Maintain professionalcompetence through attendance and participation in continuingeducation programs, seminars, and professional activities andprograms.
- Participate in care plans and other committees asdeemed necessary. Residents’ Rights Functions
- Treat Resident’swith kindness, dignity and respect.

- Help the residents exercise and/or protecttheir rights.

- Follow and adhere tofacility’s policies and procedures implementing HIPAA requirementsfor the privacy and security of protected health information.
- Protect privacy and confidentiality of information pertaining tothe resident, employee, residence, facility information andrecords.
- Use and/or disclose only minimum amount of ProtectedHealth Information necessary to complete assigned tasks.
- Reportall suspected violation of company’s HIPAA policies or proceduresto the Administrator. Infection Control
