Overview:

We’re looking fora Licensed Practical Nurse Continuous Care to join our team. Thisposition will report directly to the ExecutiveDirector/Administrator or Director of Clinical Services, but workunder the direction of the Registered Nurse to provide continuouscare visits to patients and families in a facility/unit or homesetting. The Licensed Practical Nurse Continuous Care will activelymonitor patients and report changes in their condition. Achieve palliation and management of acute medical symptomsduring brief periods of crisis Under the direction of an RN, provide patient care services tohospice patients and supportive care to the patient’s family with areasonable case load Evaluate patient/family needs and assist in the preparation andexecution of the plan of care for each patient Provide direct patient care, including administeringmedications and monitoring vital signs Provide education and support to patients and their familiesregarding their healthcare needs Report findings or changes in the patient’s condition to theRegistered Nurse, interdisciplinary team and referring physicianand document all reports

About You:

Graduate of an approved school of Practical Nursing A minimum of two (2) years’ experience as an LPN/LVN, either inlong-term care, hospice, or home health. Three to five years’ experience preferred Compassionate, kind and empathetic to our patients and theirfamilies Ability to assess and respond to the needs of patients,families and caregivers Certified in
