Summary We are seeking a Licensed Practical Nurse to join our team at ExpectCare Hospice. In this role, you will play a vital part in coordinating patient care and ensuring that our patients receive the highest quality of service. Your contributions will directly impact the lives of patients and their families during critical times. Responsibilities Assess needs of the patient and family being aware of the physical, emotional, and spiritual aspects which may influence health, well-being, and quality of life. Provide direct care to patients as ordered in the Interdisciplinary Plan of Care to maintain the highest level of comfort and quality of life. Collaborate with Registered Nurse Case Manager and interdisciplinary teams to develop and update individualized are plans that address the physical, emotional, and spiritual needs of patients and their families. Document accurate and ongoing assessment of patient status via EMR System and ensure patient chart is up to date. Document patient care reflecting nursing interventions, patient response to care, patient needs, problems, capabilities, limitations, and progress toward goals. Implement strategies for pain and symptom relief, ensuring patient comfort in the home or inpatient settings. Provide guidance to caregivers on patient care techniques, and offer emotional support to families coping with end-of life challenges. Maintain accurate and timely records of patient conditions, treatments, and responses, and communicate significant chagnes to the healthcare team. Requirements Current License Practical Nurse license in the state of practice. Hospice experience preferred Strong critical thinking skills with the ability to assess complex situations effectively. Excellent communication skills for interacting with patients, families, and healthcare professionals. Knowledge of HIPAA regulations and medical terminology is essential.
