This is a fill in spot for vacations. 36hrs a week Shifts include 7am-7pm, 7am-3pm, and 3pm-11pm shifts. This is not a set rotation. Need flexibility. Will work at least every other weekend.

Position Summary:

Under the supervision of a Registered Nurse (RN), the Licensed Practical Nurse (LPN) provides high-quality nursing care, ensuring positive clinical outcomes and patient/family satisfaction. The LPN practices within the scope defined by the State Nurse Practice Act, contributing to nursing assessments, care planning, direct patient care, and supervision of unlicensed staff.

Responsibilities & Accountabilities:

Patient Evaluation & Care Planning:

Assists in nursing assessments and contributes to individualized care plans.

Direct Patient Care:

Provides hands-on nursing care while ensuring patient comfort and safety.

Supervision of Unlicensed Staff:

Monitors and oversees patient care provided by unlicensed personnel.

Medication Management:

Conducts drug counts at the beginning and end of shifts, as well as when necessary.

Physician Assistance:

Supports physicians during rounds and patient examinations as needed.

Communication & Coordination:

Participates in shift-to-shift handoffs and collaborates with other departments for effective patient care.

Professional Development:

Attends all required in-service training sessions to enhance nursing practice.

Orientation & Training:

Assists in the onboarding and training of newly hired nursing staff.

Flexibility & Commitment:

Willing to stay beyond scheduled shifts when necessary to meet staffing requirements and patient needs.

Additional Duties:

Performs other RN-assigned tasks as required.

Educational & Vocational Requirements:

Graduate of an accredited practical or vocational nursing program. Active state licensure as an LPN. Preferred experience in rehabilitative or geriatric nursing. Current BLS/CPR certification (required for SNFs, Behavioral Centers, and applicable ALFs/ILFs per state regulations).
