The LPN will provide quality nursing care to residents, support recovery and rehabilitation efforts, and ensure patient comfort and safety under the direction of RNs and physicians.
Key Responsibilities:
Provide direct patient care including monitoring vital signs, administering medications, and assisting with daily living activities Collaborate with interdisciplinary teams to implement individualized care plans Document all care provided in accordance with facility policies and regulatory requirements Assist with wound care, catheter care, and other clinical procedures as needed Monitor residents for changes in condition and promptly report to RN or physician Support patient rehabilitation goals by encouraging mobility and independence Provide emotional support and education to residents and their families Maintain a clean, safe, and supportive environment Qualifications Valid LPN license and certification in Basic Life Support (BLS) Proficient in patient care, assessment, and monitoring Experience with catheter care and medication administration Knowledge of EHR systems and HIPAA regulations Familiarity with care plans and tube feeding techniques Preferred experience in critical care, long-term care, or assisted living settings
