Licensed Practical Nurse for Memory Care (PRN & Every other weekend) Serenity South Troy Senior Living 610 Botts Avenue, Troy, AL 36081 Overview The Memory Care LPN plays a vital role in overseeing the care and support provided to residents with memory-related conditions. This position requires a compassionate and knowledgeable individual who can manage a team while ensuring the highest standards of care are met. The Memory Care LPN will be responsible implementing programs that enhance the quality of life for residents, while also ensuring compliance with regulatory standards. Responsibilities Lead and manage the memory care team, providing guidance, training, and support to ensure high-quality resident care. Implement individualized care plans that address the unique needs of each resident. Monitor and evaluate the effectiveness of care plans. Ensure compliance with all regulatory standards related to memory care services. Collaborate with healthcare professionals to coordinate medical care, including vital signs monitoring and airway management when necessary. Conduct regular assessments of residents’ needs and progress. Provide support to families regarding memory care practices and resources available for their loved ones. Maintain a safe environment by adhering to safety protocols and lifting standards when assisting residents. Administer residents medication. Skills Strong knowledge of geriatrics and experience working with individuals suffering from memory-related conditions. Proficiency in performing vital signs assessments and understanding basic medical equipment. Excellent communication skills, both verbal and written, to effectively interact with residents, families, and staff. Ability to lift and assist residents as needed while adhering to safety standards. Familiarity with aliment management practices in memory care settings. Strong organizational skills to manage multiple responsibilities effectively while maintaining attention to detail. A compassionate approach towards caregiving, demonstrating empathy and understanding toward residents’ needs. This position is essential in fostering a nurturing environment for individuals living with memory challenges, ensuring they receive the highest level of care possible.
