Required Qualifications:
Oneyear of recent experience as an LPN, including a minimum of sixmonths in a Home Health setting as an LPN Valid LPN license incompliance with state regulations Current BLS (AHA/ARC)
Certification Preferred Qualifications:
Other certifications andlicenses may be required for this position
Summary:
A Home HealthLicensed Practical Nurse (LPN) delivers direct nursing care topatients in their homes, ensuring comfort, stability, and effectivemanagement of their medical conditions. Home Health LPNs followcare plans, administer medications, assist with treatments, andmonitor patient progress under the supervision of an RN orphysician.
Essential Work Functions:
Deliver direct patient care inaccordance with individualized care plans, physician orders, andnursing protocols Administer medications and treatments, includinginjections, wound care, and monitoring for side effects Assist ineducating patients and caregivers on proper medication use,treatment plans, and home care techniques Observe and reportchanges in patient conditions to the supervising RN or physicianfor timely intervention Collaborate with RNs, therapists, and otherhealthcare providers to ensure seamless, high-quality patient careMaintain accurate and timely documentation of all care provided,patient responses, and any concerns Ensure compliance withinfection control, safety, and home care best practices to protectpatients and caregivers Support families and caregivers, offeringguidance and emotional support throughout the care process Maintaincommunication with case managers and interdisciplinary teams toprovide coordinated patient care Strong ability to completecharting and documentation requirements while meeting visit andproductivity goals Adhere to infection control, safety, andhealthcare regulations Perform other duties as assigned within thescope of practice
Required Essential Skills:
Critical thinking,service excellence and good interpersonal communication skills Theability to read, write, & communicate in the English language isrequired Ability to read/comprehend written instructions, followverbal instructions, and proficiency in PC skills PhysicalAbilities
- Remain in a stationary position, move about, moveequipment (50-100lbs), pushing, pulling, bending InterpersonalSkills
- To work effectively with a variety of personnel(professional and ancillary) to present a positive attitude andprofessionalism Technical/Motor Skills
- Ability to grasp, finemanipulation, pushing/pulling and able to move about when assistingwith procedures and/or using department equipment. MentalRequirements
- Must be able to cope with frequent contact with thegeneral public, customers, and meeting deadlines under pressure.
Must be able to work under close supervision occasionally as wellas working without assistance from other personnel. Must be able tocontend with irregular activity schedules occasionally andcontinuous concentration to detail Sensory
