ResponsibilitiesVerify plan of care with the RNprior to each visit.
Collaborate with physician and RN frequently toensure patient needs are being met; Notify them as soon as possiblewhen patient condition changes.
Teach patients and caregiversself-care techniques.
Check vital signs.
Review all medications andperform a medication reconciliation at each visit.
Performspecialized procedures ordered by physician ( i.e. foley catheterchange).Prepare equipment and materials for treatments using theaseptic technique.
Complete documentation timely andaccurately.
QualificationsOne year of clinical LPNexperience.
Current LPN license, specific to the state(s) you areassigned to work.
Current CPR certification.
Valid driver’s license,reliable transportation and liability insurance.
