Starting at $24.00 The Licensed Practical Nurse (LPN) is a support personnel for the Intellectual Disabilities Program. This person in an integral member of the Interdisciplinary Team. The LPN provides nursing services, health care monitoring, and staff training.

Know How:

The position requires a valid LPN licensure and the ability to pass the

ABON MAS

test. A minimum of one year nursing experience is preferred. This position requires the ability to work closely with other agency personnel and the medical community, knowledge of physical assessments, and knowledge of pharmacology, particularly psychotropic medication. A valid Alabama driver’s license and vehicle insurance acceptance is required. It also requires counseling skills, (Microsoft Office), interpersonal skills, personnel and staff development skills, time-management skills, demonstrated leadership, and excellent verbal and written communication skills.

Principal Activities:

1. Evaluate physical condition of individuals utilizing examinations of vital signs and body systems.. 2. Obtain necessary information regarding individual’s health in an accurate and thorough manner to determine appropriate referrals for treatment.. 3. Provide nursing services to meet the healthcare needs of individuals as prescribed by their physicians and as outlined in Best Practices. 4. Insures services and medical records are in compliance with all regulatory agencies including but not limited to DMHMR, licensures and accreditation standards. 5. Provide input into the development of individualized support plans to achieve health-related goals. 6. Participate in special team meetings including lifestyle support planning, meetings and behavior support planning meetings. 7. Assist in scheduling medical appointments and monitoring implementation of physicians’ orders and recommended follow-up labs and/or referrals. 8. Monitor health related services through observation, review of records, and discussions with individuals served, staff, and community medical providers. 9. Provide follow-up care after illnesses, hospitalizations, outpatient surgeries and procedures, or as deemed necessary by RN. 10. Provide staff training on medical issues including medication assistance, medication side effects, infection control, seizures, lifting and physical handling. 11. Provide Medication Assistance Certification training delegation, and monitoring of MAC Workers. 12. Maintain current knowledge of physicians and health-related resources which may meet the health needs of individuals in the program. 13. Assume/share 24 hour on-call emergency duty. 14.
