Responsibilities:

• Coordinate and oversee the completion of the Minimum Data Set (MDS) assessments for all residents in a nursing home facility
• Ensure accurate and timely completion of MDS assessments in compliance with state and federal regulations
• Collaborate with interdisciplinary team members to gather necessary information for MDS assessments
• Review medical records, conduct interviews, and observe residents to gather data for MDS assessments
• Analyze assessment data to develop individualized care plans for residents
• Monitor and update care plans as needed based on changes in residents’ conditions
• Provide education and training to staff on MDS documentation requirements and processes
• Participate in quality improvement initiatives related to MDS assessments and resident care

Experience:

• Previous experience in a healthcare setting, preferably in ICU or acute care
• Familiarity with Athenahealth or similar electronic health record systems
• Knowledge of hospital discharge planning processes and primary care practices
• Strong understanding of patient care principles and practices
• Knowledge of anatomy and medical terminology
