Job Description We are seeking a compassionate and skilled Wellness Nurse (LPN) for a full-time position at Spring Meadows by Monarch located in Trumbull, CT. As a vital member of our healthcare team, you will provide holistic nursing care to our residents, focusing on their overall well-being and quality of life.
Schedule & Hours:
11:00pm – 7:00am – Sunday, Monday, Tuesday, Thursday, and Friday.
Salary:
Starting at $35/hr. Monitor the health, safety, and well-being of all residents Provide direct nursing care with an emphasis on holistic wellness Assist in training and monitoring medication administration Supervise care staff in accordance with current state regulations and community policies Maintain clinical quality assurance in line with federal, state, and local standards Conduct thorough resident assessments Collaborate with the Health and Wellness Director (RN) to ensure optimal care delivery Promote the highest degree of service to our residents while demonstrating the mission of the company Assist in the supervision of day-to-day nursing activities performed by Certified Nursing Assistants Ensure proper documentation of all nursing interventions and resident care Qualifications Graduation from an accredited School of Nursing with a current LPN license Minimum of 2 years’ experience in long-term care, assisted living, home health, or hospital setting preferred Experience working with residents with Alzheimer’s or other related dementias Strong knowledge of federal and state regulations, and nursing practices for health and wellness Proficiency in computer skills, including Microsoft Office (Windows, Outlook, Excel) Excellent communication and interpersonal skills Ability to handle multiple tasks and work efficiently in a fast-paced environment Demonstrated ability to work independently and as part of a collaborative team Current CPR certification Physical requirements: ability to stand, walk, sit, use fine and gross motor skills, reach, balance, stoop, kneel, crouch, and lift or carry weight up to 50 lbs Flexibility to adapt to changing resident needs and work schedules Empathetic and patient-centered approach to care Detail-oriented with a strong commitment to accuracy in medication administration and documentation Additional Information
