Clinical EvaluatorRN or LPN Full-Time Remote What You’ll Do:
As an ClinicalEvaluator, you will be responsible for conducting thoroughassessments of residents’ health conditions, functional abilities,and care requirements. Your evaluations will drive the developmentof personalized care plans, ensuring the highest standard of careand well-being of our residents.
Key Responsibilities:
Performcomprehensive assessments of physical, mental, and emotional healthto determine appropriate care setting Evaluate functional abilitiesand care needs through detailed observations and assessmentsCollaborate with interdisciplinary teams to develop individualizedcare plans Develop and maintain positive relationships with areahospitals and community agencies Establish and maintainrelationships with social workers, case managers and other contactsin the healthcare and payor community Educate residents andfamilies about facility services, procedures, and care expectationsMonitor and reassess residents’ progress and update care plansaccordingly Advocate for residents’ well-being and coordinate withhealthcare providers to address needs Document assessments and careplans accurately and timely If you are a skilled and empathetichealthcare professional with a passion for assessing and advocatingfor patient care needs, join our team as a Clinical Evaluator. What You’llBring:
Qualifications of a Clinical Evaluator include: Valid stateRN or LPN nursing license 3 years of clinical or case managementexperience in long-term care Experience in discharge planning andmarketing activity Compassionate and empathetic approach to patientcare Interest in the nursing needs of the aged and the chronicallyill with the ability to work with both Strong clinical skills andcritical thinking abilities Excellent communication andinterpersonal skills Ability to work effectively in amultidisciplinary team environment•: We Hire for Heart!
