Job Summary Provides timely, effective, high quality andsafe nursing collaboration for all enrollees in the program underthe direct supervision of a registered nurse. Provides coordinationof care in conjunction with the RN, BHH Specialists, Peer RecoverySpecialists, medical providers and other medical facilities asappropriate and required. Provides education (individually and ingroup format) in the areas of chronic conditions, newly diagnosedconditions, medications, health promotion, wellness & preventionincluding appropriate usage of medical services (i.e. PCP vs EDvisits). Position works closely with BHH team on a day-to-daybasis; reports to nursing services. Qualifications Completion of alicensed LPN program. Valid CT LPN license. Minimum of one years’experience in Medical/Surgical, Critical Care, Psychiatric orSubstance Abuse Treatment preferred. CPR certified within sixmonths of employment.
