Job DescriptionFacilitate communication between the patient and theinterdisciplinary team. Qualifications Licensed Practical Nursewith Valid CT LPN license One year of LPN experience; Homecareexperience preferred. Valid driver’s license and reliabletransportation required. Computer proficiency required.
