The LPN/LVN administers nursing care according to the plan ofcare to terminally ill patients admitted to the VITAS program, inaccordance with the Nurse Practice Act in the State where the VITASprogram is located.
JOB RESPONSIBILITIES
Provides direct skilledbedside nursing care.
Evaluates patient/family needs and assists inthe preparation and execution of the interdisciplinary plan of carefor each patient on the team.
Performs selected acts in the care ofthe terminally ill under the supervision of a registered nurse;such acts include the administration of treatments andmedications.
Evaluates effectiveness of treatments and monitorspatients reactions.
Reports significant findings or changes in thepatients condition to the registered nurse, Team Manager orPhysician. Documents reports in clinical notes.
Maintains a cleanand safe environment for the patient and family.
Maintain accuratedocumentation including but not limited to clinical notes oninterdisciplinary charts.
Evaluates patient/family response tocare.
Utilizes a wide variety of supportive and technical nursingskills in carrying out specific medical orders for administrationof medication, performing specific treatments, monitoringdiagnostic, therapeutic, and supportive measures.
Assists theinterdisciplinary team in the designing and delivering ofinterdisciplinary plan of care.
Establishes therapeutic relationshipwith patients/families and invites their participation in the planof care to ensure, as much as possible, their being able tofunction as independently as possible.
Initiates and performsnursing protocols based on patient’s needs relating to physicalcomfort and mental well-being which include, but are not limitedto, positioning, mobilizing, bowel and bladder elimination, skinand wound care, personal hygiene and communication.
Participates inpatient/family teaching during daily care relative to understandingthe illness and its treatment; maintaining of activities of dailyliving; self-care and self
- medication, prevention and recognitionof complications and preparation for discharge and follow-up.
Plansand carries out a program of professional growth including, but notlimited to, participating in regularly scheduled in-serviceprograms and appropriate community educational offerings.
Cooperateswith education and research activities as assigned.1. Participatesin Nurse LPN/LVN on-call rotation/schedule as assigned in order tomeet the needs of patients and families.2. Is willing to work onflexible schedule, i.e., weekends or rotating shifts.
JOBREQUIREMENTS
Must have and maintain transportation to be used forwork with required liability insurance and license.
QUALIFICATIONS1.Graduate of an approved school of Practical/Vocational nursing.2.Currently licensed to practice nursing in the state where the VITASprogram is located.3. At least one year of satisfactory nursingexperience, at least one year of which is an LPN/LVN in a hospitalon an adult medical surgical unit.4. Equivalent experience may beconsidered.
