The ideal candidate will provide compassionate care to patients in home care setting.
Responsibilities:
- Current LPN license
- Strong written and verbal communication skills
- Excellent work ethic in both independent and collaborative settings
- Dedication to patient success
- Knowledge of Wellsky/Kinnser is a plus
- Administer medications and treatments as prescribed by healthcare providers
- Perform medical documentation accurately and timely
- Conduct IV infusions and manage IV lines
- Monitor patient conditions and report changes to the healthcare team
- Collaborate with physicians and other healthcare professionals to ensure comprehensive patient care
- Several positions available

- Valid LPN license in the state of practice
- Experience in home care required
- Strong understanding of medical documentation practices
- Excellent communication and interpersonal skills
