Job Description
Licensed Practical/Licensed Vocational Nurse (LPN/LVN) provides direct bedside care to terminally ill patients following the care plan and under RN supervision. Performs treatments, medication administration, and evaluates patient responses. Documents clinical notes, monitors patient condition, and reports significant changes to the team. Engages in patient/family education, supports comfort measures, and collaborates in care planning to promote independence and well-being. Participates in on-call rotations and flexible scheduling, including weekends and evenings. Requires valid state license, relevant experience, and reliable transportation.
