QUALIFICATIONS
1. Health – Ability to meet performance requirements. 2.
Education, Experience and Skills:
- Graduate from an approved school of nursing, College or University.
- Must have a current, unencumbered license to practice nursing as a Registered Nurse/Licensed Practical Nurse in Florida.
- Participates in continuing education so that he/she has knowledge of current trends and developments in nursing and maintains current license.
- Minimum of two years of RAI and Care Plan experience, preferably in a Skilled Nursing Facility (SNF).
- Must have knowledge of MDS 3.0 including Assessments, CAA’s and Care Plans.
- Conduct each Assessment and Care Plan with the appropriate participation of health professionals.
- Ability to organize work procedures, assume responsibility, and tactfully interact with other disciplines.
- Must be able to communicate well with any member of the Interdisciplinary Team.
- Collaborate with the IDT to provide guidance on appropriate and precise charting for the medical record.
PERFORMANCE REQUIREMENTS
1. Must be able to sit, stand, bend, twist, lift and move intermittently throughout the day. 2. Must be able to lift, push, pull and otherwise move a minimum of 50 pounds. 3. Able to see and hear adequately in order to respond and interact with other staff and families. 4.
