LPN FLORIDA HOMECARE SPECIALISTS

The Villages, FL Responsibilities:

• This position will cover Sumter County, Lake County to The Villages and surrounding areas.
• Implements plan of care initiated by the registered nurse.
• Monitor and record patient vital signs
• Provides accurate and timely documentation consistent with the plan of care.
• Assists the registered nurse or physician in performing specialized procedures and duties, which include preparing equipment and materials for treatments, observing aseptic technique as required, and assisting the patient in learning appropriate self-care techniques.
• Collaborate with healthcare team members to develop and implement patient care plans
• Educate patients and their families on proper healthcare procedures and self-care techniques
• Document patient information in electronic medical records system (Cerner)
• Assist with discharge planning and coordination of follow-up care
• Attend case conferences to get and provide input that will best benefit the patient

Licenses and Requirements:

• Valid FL LPN license
• Valid FL Driver’s License
• Current CPR card
• AHCA Background Screening Report
• IV Certification
• Either of the following: 2 Step TST, chest XR Report or TB lab test report (Quantiferon Gold)
• Doctor’s Certificate of Good Health Benefits (For Full-time Position):
