• Home visits in assigned service area:
• Manatee County, FL
• The
• Per Visit Home Health LPN
• provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols.
• Responsibilities include:
• + Delivery of patient care to assigned patients, provides care for clients as ordered including IV infusions, infusion pumps, and tube feedings under the supervision of a Registered Nurse + Identifies, implements, and maintains appropriate teaching plans for patient and caregivers under direction of a Registered Nurse + Follows plan of care and collaborates with Registered Nurse regarding patient response to treatment and change in status + Utilizes nursing knowledge in the administration of medication and evaluation of effectiveness when applicable.

• Required Certifications and Licensures:

• + BLS (Basic Life Support) + LPN (Registered Nurse) License + Driver’s License State of Florida (Florida driver’s license is required within 30 days of hire)

• Education:

• + Required Associate’s or Diploma Nursing

• Experience:

• + Required 1 year Licensed Practical Nurse +
• Preferred 1 year Home Care
