What You’ll Do As our MDS Coordinator , you will play a key role in optimizing resident care by leading the Resident Assessment Instrument (RAI)/Minimum Data Set (MDS) process.
Key Responsibilities:
Coordinate and manage resident care plans based on RAI/MDS data Collaborate with interdisciplinary teams to ensure care plans reflect each resident’s needs Ensure timely, accurate MDS assessments and data submission Analyze Quality Measures (QM) and Quality Indicators (QI) for ongoing improvement Support training and compliance initiatives across the care team What We’re Looking For Licensed RN or LPN in Florida Minimum 1 year of MDS Coordinator experience Strong understanding of long-term care regulations Excellent communication and organizational skills Why Join The Meadows?
