LPN Home Health Ready Care Home Health, LLC Hialeah, FL 33014 Overview We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients in various settings, including outpatient and inpatient environments. As an LPN, you will play a vital role in patient care, collaborating with registered nurses and other healthcare professionals to ensure the best outcomes for our patients. Duties Administer medications and treatments as prescribed by physicians. Perform venipuncture and assist with sterile processing of medical equipment. Engage in discharge planning to facilitate smooth transitions for patients leaving care facilities. Provide pain management support and monitor patient comfort levels. Assist in the care of patients undergoing dialysis and other specialized treatments. Support memory care initiatives for patients with cognitive impairments. Implement airway management techniques as necessary for patient safety. Document patient progress and communicate effectively with the healthcare team. Participate in patient education regarding health management and wellness. Qualifications Current state licensure as a Licensed Practical Nurse (LPN). Proficiency in sterile processing techniques. Strong skills in venipuncture and patient assessment. Knowledge of discharge planning processes and pain management strategies. Familiarity with inpatient nursing practices is a plus. Excellent communication skills, bilingual spanish-both verbal and written, with a focus on patient-centered care. Ability to work collaboratively within a multidisciplinary team environment.
