Position OverviewThe Licensed Practical Nurseis an essential part of the team responsible for providing anddocumenting skilled nursing care in accordance with the developedcare plan and physicians’ orders for each individual patient whileadhering to confidentiality standards and professional boundariesat all times.
Essential Job Functions Deliver quality patient care in compliance with physicianorders under direction of their Registered Nurse teammate. Performs the continuum of hands on client care throughdocumentation and timely communication with the client’s physicianand other caregivers Recognizes changes in patient needs and responses requiringintervention and implements care to prevent risk or reducerisk. Educate clients and their family members based on client’sspecific needs. Provide care in patients home using a variety of skills such asphone triage, patient education, observation/assessment, woundcare, infusions, catheter care, PICC line dressing changesetc.
