This position will beconducting patient home visits in the Downtown Orlando, Pine Hills,West Orlando, Ocoee, Winter Garden, Windemere area which includesbut is not limited to the following zip codes 34760, 34787, 34734,34786, 34761, 32818, 32835, 32819, 32811, 32808, 32810, 32714,32804, 32805, 32839, 32806, 32801 The role you’ll contribute:

Administer nursing care to patients in accordance with thephysician’s order and under the direction and supervision of aRegistered Nurse. Provides intermittent, individualized nursingcare, teaches and supervises the patient and family members orcaregivers to provide continuity of care and to assist inimplementing comprehensive care. Delivers high quality nursingcare, performs technical nursing skills with a high degree ofproficiency, and promotes the team approach in the delivery ofpatient care. Executes physician’s orders and notifies RN/CaseManager of necessary updates or changes in condition. Completesdocumentation according to Agency Policy and Regulatory guidelinesand performs home visits as ordered. Continued employment with theagency is contingent upon your ability to perform job duties asoutlined in this job description. The value you’ll bring to theteam: Comply with all department policies, procedures, practicesand standards of care. Provide direct patient care responsibilitiesfor assigned patients in an efficient, organized manner under thedirection of the RN. Performs treatments within their scope oftraining and administers medications to assigned patients. Performsdata collection specific for the age of the patient and contributesto their plan of care. Report pertinent observations to attendingphysician and to RN, implements the change in the patient care planas directed by physician. Communicates the changes to theappropriate members of the home health care team. Receives,verifies and implements physician orders (orders taken by the LPNare co-signed by an RN). Demonstrates effective communicationskills with all team members, patients and families. Maintainsclinical record information in accordance with establishedstandards and within established time frames. Qualifications Theexpertise and experiences you’ll need to succeed:
