//bhconnections.org Behavioral Health Connections (BHC) in Orlando is seeking a dynamic and talented LPN Full Time Day Referral Management that provides mental health assessments for contracted emergency rooms and/or hospitals, recommends the appropriate level of treatment based on the Assessor’s assessment of risk factors, and triages patients to appropriate services.

Job Duties/Responsibilities:

Perform all aspects of the hospital’s assessment and referral process. Service coordination by matching patients with appropriate mental health services and providers within UHS, considering factors such as specialty, location, and systems complexities. and insurance coverage. Responsible for providing and assisting patients with resource navigation, assisting patients in understanding their treatment options and guiding them through the healthcare

EDUCATION

Nursing Degree from Vocational/Technical School, preferred Associates Degree in Nursing

LICENSURE/CERTIFICATION

Licensed LPN State of Florida or compact license Current CPR, if required by a customer hospital. (“Customer hospital” means the hospital to which BHC is contracted to provide mobile assessment services.)
