As the MDS Coordinator, you will be responsible for ensuring the completion of the Resident Assessment Instrument (RAI) process to ensure the accurate and timely completion of patient assessments in accordance with Medicare, Medicaid, and other payer program requirements as well with all applicable regulatory guidelines. Ansley Cove Health & Rehabilitation is a 39 Bed SNF in Maitland, FL. Duties Ensures MDS and related assessments are completed in timely manner and in accordance with state and federal guidelines as well as company RAI policies and procedures. Monitors initial, quarterly, and annual plans of care to ensure compliance with regulatory guidelines and company policies. Ensures quarterly reviews and annual care plans are completed timely, using an interdisciplinary approach that reflects the needs of the patients. Assists the Director of Nursing with monitoring to ensure that a care plan is initiated on every resident upon admission to the facility. Assures completion and accuracy of the RAI process from MDS through implementation of the care plan. Assists Director of Nursing with identification of significant changes, physician orders, and verbal reports to ensure that MDS and care plans are reflective of those changes. Ensures all MDS information and care is delivered as outlined in the care plan and is supported by appropriate documentation. Follows up with staff as necessary to assure compliance to standards of documentation. Provides training pertaining to completion of MDS and RAI process as appropriate. Provides interdisciplinary schedule for all MDS assessments and care plans as required OBRA and PPS. Coordinates scheduling and notice of resident care planning conferences and assures communication of outcomes and/or concerns to relevant parties. Determines appropriate discharge dates for Rehab patients to ensure safe discharge and participates in the patient discharge process. Completes final MDS accurately and submits resident assessment data to appropriate state and federal government agencies in a timely manner. Implements auditing procedures to ensure accuracy of MDS and related assessments.
