Role:
Under RN supervision, the Per Diem – LPN is responsible forproviding nursing interventions and teaching skills to maximizepatient comfort and to enhance the quality of life to thepatient/family unit.
Qualifications:
- A high school diploma or G.E.D. or an equivalent combination ofeducation and experience
- Current LPN licensure in the state of employment
- IV certification required at time of hire beginning November 10,2024. Employees in this job prior to November 10, 2024 will berequired to obtain certification by May 10, 2025.
- Active BLS for healthcare professionals from the American HeartAssociation or Red Cross.

Job Responsibilities:
- Under the direction of an RN, provides nursing tasks to patientsand families such as continuous care, dressing changes, catheterchanges as identified in the plan of care developed by theInterdisciplinary Group (IDG).
- Demonstrates clinical skills regarding application of Companypolicies and procedures such as: medication administration, symptommanagement, and patient/family relationships.
- Observes/monitors patients for alterations in body systems, painand symptoms and reports to RN.
- Observes and reports signs of adverse reactions and responses totreatments and reports to RN.
- Reports findings to physicians, APRNs and physician’s assistantsfollowing collaboration with RN.
- Performs venipuncture for blood specimens.
- Performs Central Venous Line (CVL) site observation/inspection;venous access device dressing, cap change and flushing venousaccess device; IV medication and fluid administration, IV pushes(per procedure) and venipuncture. May not make any changes toinfusion pump including, but not limited to, rate changes and IVbag changes.
- Documents per policy and procedure in medical record.
- During times of emergencies (i.e. Hurricanes, etc.), the LPN maybe required to report to work at a location designated by thecompany, to ensure continuity of services. This may includereporting to work ahead of your scheduled date/time due to plannedlock down of unit, and staying overnight(s) based on duration ofemergency
- Performs other duties as assigned.
Some locations may require:
- Cross training to Hospice House positions of equallicensure/certification and scope when census calls for flexiblestaffing.
