Case Manager – Licensed Practical Nurse (LPN)

Hours:

M-F 8am to5pmContract (ongoing)

Position Overview:

The Case Manager LPN isresponsible for ensuring the seamless coordination of care forpatients across inpatient and outpatient settings. This roleinvolves working closely with patients, families, caregivers, andhealthcare teams to optimize treatment plans and improve healthoutcomes within the parameters of established contracts andpatients’ health plan benefits.

Key Responsibilities:

Ensure continuity of care by utilizing appropriate resourceswithin the inpatient and outpatient settings. Perform care management functions either on-site ortelephonically as required. Collaborate with the care team and Primary Care Physician (PCP)to coordinate patient care. Meet with patients, families, and caregivers to discuss andimplement treatment plans in accordance with the Transitions ofCare program. Identify and facilitate follow-up care for high-risk patientsin acute care settings, skilled nursing facilities, custodial, andambulatory environments. Consult with physicians and care team members to ensuresuccessful implementation of patient care plans. Coordinate treatment plans with healthcare professionals andtriage interventions based on team member expertise.

Qualifications& Experience:
Licensure:

Current and valid Florida LPN license.

Certification:

IV Certification required.

Experience:

Minimum of 1+ years in an acute clinical care setting. Previous experience in care management, utilization review, ordischarge planning. Familiarity with HMO processes and guidelines.

Preferred Skills:

Strong communication and interpersonal skills for patient andfamily engagement. Ability to coordinate care across various medicalsettings. Proficiency in electronic medical records (EMR) systems andcare documentation. Problem-solving skills with a patient-centered approach.
