The Licensed Practical Nurse (LPN) in Home Health provides basic nursing care to patients under the supervision of a Registered Nurse (RN). The LPN plays a critical role in monitoring patient health, administering medications, and supporting the overall patient care plan. This position requires strong organizational skills, clinical judgment, and a compassionate approach to care, as well as the ability to work independently in patients’ homes. Minimum Qualifications Graduate of an accredited LPN/LVN program. Current Licensed Practical Nurse (LPN) license in the state. Minimum of 1 year of clinical nursing experience, preferably in home health or a related field. Ability to follow care plans and clinical instructions accurately. Proficient in electronic health records (EHR) systems. Strong communication and interpersonal skills. Current CPR certification. Job Responsibilities Assist in the development and implementation of individualized patient care plans. Provide basic nursing care, including wound care, injections, and administering medications as directed by an RN or physician. Monitor and document patient health, reporting changes to the supervising RN or physician. Assist with activities of daily living (ADLs) such as bathing, dressing, and feeding, as necessary. Educate patients and families on basic care techniques, medications, and disease management. Support the Registered Nurse in coordinating with physicians and therapists to ensure comprehensive patient care. Maintain accurate and timely documentation in electronic health records. Comply with home health regulations and company policies. Ensure compliance with the organization’s Code of Conduct and all applicable laws, including HIPAA regulations for the protection of sensitive information. Perform any additional duties as assigned to meet the needs of the department and organization.
