The Home Health Nurse, LPN/LVN enables patients to stay in their homes by providing Skilled Care in the patient home, supporting patients and their families. Provide professional nursing services under the direction of a RN. Provide skilled nursing interventions in the treatment of the patient/clients’ illness, rehabilitative needs, and preventative care. Utilize a holistic approach in the provision of problem specific therapeutic interventions, teaching, and training activities in accordance with the plan of care. Apply knowledge and skills in accordance with accepted standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes. Maintain the highest standards of professional conduct in relation to information that is confidential in nature. Provide skilled nursing interventions in the treatment of the patient/clients’ illness, rehabilitative needs, and preventative care Administer medications and monitor patients for side effects or reactions Document any changes in patient conditions and report on changes Maintain accurate records of patient care and discuss observations with the supervisor Qualifications Currently licensed Registered Nurse (LPN) in state of employment Previous home health experience as a LPN is preferred Excellent interpersonal and client care skills Valid driver’s license First aid/CPR certification required
