This position requires a compassionate and skilled nursing professional who can deliver high-quality care while collaborating with a multidisciplinary team. The Wound Care Nurse (RN/LPN) will be responsible for developing individualized care plans, educating patients on wound care practices, and ensuring adherence to best practices in wound management. Duties Conduct comprehensive assessments of patients’ wounds and overall health status. Develop, implement, and evaluate individualized care plans tailored to each patient’s needs. Perform wound debridement and dressing changes using appropriate techniques and sterile procedures. Educate patients and their families on wound care management, prevention strategies, and lifestyle modifications. Collaborate with healthcare providers to coordinate care for patients requiring specialized treatments. Document all patient interactions, assessments, and treatment plans accurately in medical records. Stay current with advancements in wound care technology and techniques through ongoing education and training. Participate in quality improvement initiatives related to wound care practices within the facility. Skills Must be experienced. Knowledge of Senior care principles to effectively manage wounds in elderly populations. Skills in Sterile processing to maintain a safe environment during procedures. Competence in managing aliment issues that may affect healing processes. Experience in Acute pain management strategies for patient comfort during treatment.
