About This Role:

Assesses, plans, and coordinates comprehensive care for medically complex patients to ensure quality, cost-effective outcomes, identifying gaps through patient data analysis and nursing expertise. Conducts outreach to assigned Evara Health patients, providing education and support to encourage proactive health decisions and close care gaps. Collaborates regularly with providers, clinical teams, and program managers to review cases, maintain appointment schedules, and align care strategies. Maintains accurate and timely documentation (within 24 hours), including case notes, outcome measures, and reports, in compliance with agency and payer guidelines. Utilizes data-driven strategies, including HEDIS and Meaningful Use reports, to support performance metrics, improve patient outcomes, and inform clinical and billing processes.

Education & Experience:

Graduate of an accredited LPN or RN program Minimum two (2) years of experience as an LPN or RN in healthcare Minimum one (1) year of experience as a Utilization Review Nurse or Case Manager

Active and unencumbered LPN or RN License in the
