Position Type Full-Time Empath Health is currently seeking an LPN to join our team. Monday – Friday 8am to 5pm Under the direction of RN case manager, provides clinical, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition as needed. Provides support to family/caregiver reporting psychosocial/spiritual needs to team.
Requirements:
Applicants must be a graduate of an approved nursing program and own a current Florida LPN license.
Minimum one year experience in an acute care setting, providing direct patient care; previous experience as a hospice LPN preferred.
