CARE NURSE
(LPN/RN) Washington Rehabilitation & Nursing Center is seeking an experienced Wound Care Nurse (LPN/RN) to join our team. Job Summary:
The LPN/RN Wound Care provides care that will quickly and safely heal various types of wounds, including surgical openings, ulcers, bedsores, feeding tube sites, and abscesses. The wound care nurse will access the wounds, create and implement treatment plans, monitor infection and signs of various infections and clean wounds.
Essential Job Functions:
Maintain a Comprehensive care plan for skin impairment- updating as needed to include any change in treatment, improvement and/ or decline Monitoring vascular/arterial/surgical wounds and document appropriately- treatment, comprehensive care plan, coordination follow-up when indicated Ensure weekly wound measurements are documented. Maintain facility wound tracking report Maintain skin care policy Monitor residents with devices to ensure skin maintenance. Ensure devices are identified on plan of care and worn appropriately via audit tool/ rounds Implement physician orders accurately within appropriate time frames. Maintain ongoing communications with physicians concerning resident care. Administers wound care medications per policy and licensure regulations and completing the appropriate documentation. Collaborate amongst interdisciplinary team to encompass all aspects of care to promote would healing. Review investigation for all new skin opening related to pressure. Knowledgeably and competently deliver quality nursing care to patients. Identify changes in patients’ conditions, intervene appropriately and report changes to Director of Nursing. Prioritize nursing actions including reflecting patient needs, acuity and desired outcome. Knowledge of practices and procedures as well as the laws, regulations, and guidelines governing functions in long term care. Assist surveyors and inspectors to facilitate regulatory compliance, as per the direction of the Director of Nursing. Work with nursing team to educate CNAs as to proper positioning, training, and skin care. Other Responsibilities Oversee durable medical equipment related to the prevention and management of wounds. Support central supply serving as the clinical expert for skin, would and incontinent products. Participates in the on-call rotation.
Wound Nurse Qualifications:
Valid LPN/RN license in the state of Florida Valid
CPR/ BLS
certification Knowledge of nursing principles and practices Excellent communication and interpersonal skills Ability to work independently and as part of the team Attention to detail and ability to multi-task Prior wound care experience preferred in long term care or a related setting. Must have knowledge of national would guidelines and wound products. Strong clinical assessment and critical thinking skills.
