Location:
Servicing Gordon, Floyd and Bartow. The role you’ll contribute: TheLicensed Practical Nurse (LPN) is responsible for the delivery ofcoordinated nursing care for a patient and/or assigned group ofpatients within a patient care unit and for the day-to-daymanagement and supervision of the related clinical activities.
Under the direction of a Registered Professional Nurse (RN), theLPN functions to provide and ensure the completion of activities ofvarious levels of assigned nursing and ancillary personnel for thatpatient or group of patients. The LPN practices within the State,Facility and regulatory guidelines. The home health care LPN’spurpose is to deliver a community based approach that supports themission and continuum of care of Gordon Hospital and is aspecialized form of health care dealing with the delivery ofskilled medical care to the patient and supportive care to thefamily and/or caregivers in the patient’s home environment. It isdirected towards teaching the patients/families/caregivers on howto manage the illness at home, assist the patient and familythrough the rehabilitative process, and teach them how to return tothe highest level of functioning possible to maintain independencein their own environment. It is individualized, holistic, patientcentered care, which acknowledges caring for the patient and familyas a unit of care and recognizes the need for the patient to be anactive participant in his/her care. The value you’ll bring to theteam: Optimally provides the delivery of patient care, documentingappropriately and accurately on the visit tools so that the AdverseOutcomes Report does not reflect quality of care issues and properreimbursement is obtained while complying with all departmentpolicies, procedures, practices and standards of care. Communicatespatient’s rehabilitation status with other health careprofessionals to facilitate interdisciplinary continuity of careand appropriate discharge planning. Identifies continuingeducational opportunities for resolution of knowledge deficitsand/or for professional growth and demonstrates the ability toevaluate suggestions and criticisms objectively and whenevernecessary undertakes to change his/her behavior or seek furtherguidance. Establishes professional goals in collaboration withsupervisor annually and demonstrates compliance with an action planto remediate deficiencies in performance identified by supervisor.
Recognizes the influence of personal and cultural values onperceptions regarding health care and their impact on the deliveryand quality of care.
