JOB SUMMARY
The
LPN AND/OR MEDICAL ASSISTANT
provides accessible, coordinated, and patient-centered care and is managed by the primary care providers. This position allows patients to have a more active role in their health care and is associated with increased quality improvement and patient satisfaction. The
LPN AND/OR MEDICAL ASSISTANT
is directly responsible for providing direct care within his/her scope of practice. Completed work need only a general review by the physician or provider for appropriateness and conformity with established policies/procedures. Observes for physical and/or emotional changes in patient’s condition, documenting noted changes and reporting these observations to the physician/provider. The

recognizes and considers emotional, cultural, socio-economic, and age-related factors of the patient.
RESPONSIBLITIES AND AUTHORITY 1.
Triages patients under the direction of the Medical Director according to policy Determines appropriate need of appointment and provider Refers questions or concerns to provider Assists patient in scheduling an appointment if appropriate 2. Prepares room and equipment for patient Cleans room from previous patient Assembles correct instruments, equipment & supplies for treatment or exam 3. Prepares patient for treatment & exam Obtains medical information and documents in ECW to include: complaint, allergies, previous illness, past medical and social history Takes B/P, TPR, BMI, height, weight Documents any patient questions or concerns for provider to note in ECW Discusses and updates Self- Management Goals from last visit Completes any nursing standing order protocols including preventive vaccines Completes risk assessment screens per protocols 4. Assists provider during treatment or exam Observes patient closely during treatment or exam and reports any changes to the provider Refers questions/concerns to the provider for clarification Relays instructions to the patient/family as directed by the provider 5. Performs Procedures as ordered by the physician/provider Spirometry EKGs Injections Immunizations BP Checks Dressing Changes Draw blood as needed 6. Patient Centered Medical Home Assist in patient compliance Discusses with patients the importance of preventive screens such as paps and PSAs Reports on high-risk, complex patients Monitors for ER/hospital/specialist visits Through discussion with patient links to community resources- referral to Outreach Dept. Documents care as directed by QI Coordinator to satisfy preventative measures 7. Other Duties as Assigned Serves as preceptor in orienting, educating, and training less experienced Nursing Assistants Understands and supports goals of PCMH, Meaningful Use, Quality Assurance Program Maintains quality, safety, and infection control standards Assists with telephone nurse duties in times of need (LPN’s only)
QUALIFICATIONS, KNOWLEDGE, SKILLS
Current
LPN AND/OR MEDICAL ASSISTANT
certification/license Current Basic Life Support (BLS) certification Minimum (2) years medical experience required Previous outpatient, primary care experience required Previous electronic medical record experience preferred Demonstrated organizational and problem-solving abilities Prior performance review documents he/she requires minimal direct supervision of job duties Knowledge and ability to recognize urgent or emergent situations Understanding of patient motivations and reactions in order to work effectively with patients & family Maintains annual competencies/nursing skills checklist Ability to educate patients/families and to provide instructions at the patient’s level of understanding
TYPICAL PHYSICAL DEMANDS AND WORKING CONDITIONS
Work is performed in a typical outpatient, medical setting. Requires manual dexterity to input patient data into electronic medical record. Requires corrected vision and hearing to normal range. Requires occasional overtime to complete tasks for the day. Requires working with patients of various ages and medical conditions which may include: pulling, kneeling, lifting and assisting patients who are physically handicapped at various stages.
