As an Assessment Nurse (AN) with our Medicaid waiver program, you’ll use your clinical skills to help older adults and adults with disabilities thrive safely at home . What You’ll Do Conduct in-home clinical assessments and reassessments Determine levels of care and develop person-centered care plans Work together with doctors, families, and case managers Coordinate Medicaid services like personal care, home health, and more Ensure quality, ethical care across you assigned client base Serve as a trusted advocate and guide to individuals and their caregivers What You’ll Need Active LPN or RN – License in the State of Georgia Multi-state license is preferred – e.g., COMPACT (NLC) At least 2 years in geriatrics, long-term care, chronic disease, or community nursing Compassion, clinical judgment, and dedication to doing what’s right Strong documentation and communication skills Reliable transportation and willingness to travel locally Why Join Us?
