In this position… Provide nursing care, emotional support, patient and family education regarding disease process, self-care techniques, prevention strategies, clinical nursing observations, and documentation for Home Health patients. Provides nursing services as ordered by the physician and under the direction of the RN. Reports subjective and objective patient findings to the RN. Under the direction of the RN, obtains physician’s orders and ensures orders exist for each visit made, Provides necessary skills to report changes appropriately; documents pertinent information and care rendered to the patient to ensure continuity of care. Demonstrates knowledge of infection control standards and techniques as per VNHS policy and procedures and consistently practices these techniques. Do you have…. LPN Graduate from an accredited school of nursing or program Completion of an IV training program or certification in IV therapy Experienced in advanced wound therapies Current license by the Georgia State Board of Nursing. Minimum of one – two (1-2) years’ experience in a Medicare-certified home health agency is preferred. Minimum of two (2) years as an LPN Current valid Driver’s License, proof of auto insurance, and a reliable vehicle
