JOB SUMMARY

The Continuous Care LPNmonitors patients and reports changes in their condition. Dutiesinclude administering treatment under the supervision of RegisteredNurse Case Managers (RNCMs), updating patient files, andinstructing families on patient care.

MAJOR JOBFUNCTIONS

Promote and practice the Sage Family of Companies philosophyand mission of, “Providing the most compassionate, comprehensive,and qualified care to people with a life-limiting illness to servethem through their individual journey with dignity, respect, andintegrity.” Provides nursing services to patients under the supervision ofRNCM. Travels to patient locations to administer bedside care forcrisis. When crisis is under control, continuous care will then bestopped. Assesses treatment plans and coordinates patient care withinterdisciplinary health professionals. Measures and records vital signs, including blood pressure,heart rate, and pulse oximetry. Administers medication for pain or symptom managementmedication as required. Evaluates patients’ responses to treatments and medications, aswell as facilitates appropriate interventions when required. Instructs families and caregivers on patient care and addressesconcerns in a sensitive manner. Updates and maintains patient files and medical records. Adheres to best practices in nursing and pursues continuedprofessional development. Perform other duties and responsibilities as assigned.

REQUIREDEDUCATION/EXPERIENCE

Two (2) years’ experience in nursing. Active license as a Registered Nurse. CPR certification. Hospice Experience Must possess a valid driver’s license, documentation of validvehicle insurance, and reliable transportation.
